At present, Brad only sees PeaceHealth patients via telemedicine who have a Primary Care Provider (PCP) with PeaceHealth Medical Group. The best way to access Brad is to have your PCP complete a referral to Adult Behavioral Health Services that specifically refers you to see “Bradley Shumate” for counseling services. Once you know that the referral has been submitted, please contact New Patient Scheduling 458-205-6426. From there, you will be scheduled to complete intake paperwork and a Mental Health Assessment with our Connections Clinic and then scheduled to see Brad should you request him. Questions?
